OKLAHOMA LIVING CHOICE PROGRAM

The process of returning to life out in the community can be challenging. It is crucial that services be available to ensure a successful transition. Depending on the needs of the individual, Living Choice services may include assistance with funding a new residence, home-delivered meals, transportation, skilled nursing, therapy services, personal care, medication management, and transitional funds to help facilitate the move.

To qualify for the program, individuals must be at least 19 years old and qualify for SoonerCare for at least one day prior to transition. They must have lived in an institutional setting for at least 90 consecutive days and voluntarily want to transition back into the community, and they must be willing to play an active role in their personal care plan. Anyone can refer a potential candidate for the Oklahoma Living Choice Program. Individuals may complete a referral on their own behalf, or a referral may be made by a family member, physician, or other interested party. Simply complete the online referral form at www.okhca.org/LTC. You will need the name of the person you wish to refer, their address, phone number and their SoonerCare/Medicaid ID number.
